Spiro is a life saver! I only had minor acne issues, mostly resolved by Benzaclin in my youth, until I turned 25 and then from 25 to 27 I struggled with increasingly worse acne. Face washes didn’t work, benzyl peroxide/benzaclin stopped helping, natural remedies (ie oil cleansing) didn’t work, nothing made a difference whatsoever until spiro.
I have aura migraines so I can’t take birth control with estrogen to correct the hormonal imbalance causing my acne. I had to request spiro from my doctor, since they usually rather treat with antibiotics or accutane (which is ridiculous considering spiro is natural and much less harsh than those drugs). Within 3 weeks my acne had completely stopped forming.
I take 25mg and have been for about 3 years now. I still get the odd little whitehead here and there, but I have not had a legitimate pimple since I started taking spiro. I did have some very, very minor nausea/dizziness in the first month of taking spiro, but only if I took it on an empty stomach. 25mg seems to be a low dose, people talking about bad side effects seem to have been on doses 100mg or higher. That is extreme, and I think if I’d started out that high I would have had significantly stronger side effects. Luckily my doctor wanted to start at a low dose and increase only if necessary, and it didn’t end up being necessary!

I’ve been struggling with mild to moderate acne for almost 15 years. I’ve tried all the acne products/medications under the sun except for accutane (a line I refused to cross). I’ve tried benzyol peroxide, bhas/ahas/chemical peels, retin-a, antibiotics, anti-fungal treatments, laser treatments, natural remedies (spearmint), birth control pills, etc. you name it I probably have tried it. For all of them it always started out promising but after a month my acne came back again as if it became resistant to the treatments. From all that I’ve tried the only thing that worked long term was birth control pills (YAZ specifically). My skin was clear when under Yaz and I was happy. However, after reading up on all the potential side effects, I stopped taking it and the acne came back again but much worse than before.
I started having hundreds of closed comedones all over my face with cystic acne along my chin line. I had so much acne scars and hyperpigmentation from acne that healed and then having new acne that would immediately growing on top of it. For me it wasn’t even having clear skin but maintaining what I had without making it worse. 6 months ago I tried zinc pyrithione thinking I may have fungal acne since nothing else seemed to work and I had the worst breakout of my life. My whole face, chin and neck erupted in painful cystic acne, it got so bad that even wearing a turtleneck in the winter was painful. I thought this was it that I would never have normal skin and either have to go back to BCP or live with this terrible acne for the rest of my life.
That’s when I decided to see a dermatologist. She recommended I try spironolactone. At that point, I had nothing to lose. I started with 25mg for two months. It did nothing for my acne except my skin became less oily and I had more frequent trips to the bathroom. Then she bumped me up to 50mg for a month. My skin was still not clearing up. I also noticed some dizziness and fatigue. On the fourth month she recommended that I alternate between 50mg and 25mg every other day to ease my dizziness and that was the magic formula. Into the fourth month I noticed my skin started clearing. I had so little confidence with my skin that I didn’t even notice the changes until my mom bought it up. Now at the fifth month, I can say that I still have acne but leaps and bounds better than what I started with. I’m 90% clear and seems to be getting better with each day. I still get a few pimples here and there during time of the month but now they disappear within days. I can’t even remember the last time my skin has felt so smooth. For all the gals out there who are still struggling and want to throw in the towel, DON’T! Try spironolactone for least 3 months! You may be surprised!
One other thing I want to end is that throughout this whole journey I noticed that my acne is also triggered by certain food specifically dairy and sugar. After reducing my intake of both I noticed significant clarity in my skin.

I started taking this medication in 1999. Except for a few years off, I have taken it ever since. My current dose is 100mg/day. I never really had a face full of acne and I do not have any scarring, thankfully. However, I had frequent cystic acne and there was always something coming and/or going on my face. I was long past my “acne years” but my skin did not get the memo, even after two courses of Accutane. I read about spironolactone in a book called “The Good News About Women’s Hormones” and I immediately made an appointment with my dermatologist. Taking this medication affected how I felt about myself and my appearance. Not having to constantly worry about my now essentially perfectly clear skin gave me the time and confidence to focus on other things, including going back to school and starting a new career that I really enjoy. I have never had any side effects from this medication. However, they are possible so women who would like to try it should visit a dermatologist to find out if it is an option for them. I have an annual blood draw to check my electrolyte levels (it is a potassium-sparing diuretic), but my results have always been fine. It is a relatively cheap medication that has always been covered by my insurance. Discovering this medication was life-changing for me, and any time I see a woman with cystic acne it is very hard for me not to pull them aside and tell them about spironolactone (but I don’t, because I know that would be weird).

Saved my face. It’s not a total miracle cure – I still get the odd pimple, but the constant parade of zits has vanished.
I’m allergic to all formulations of the pill form, so I have to take the liquid form, which is a bit more of a pain than popping a pill but still manageable. My pharmacist recommended that I get the manufacturer’s coupon so that she could double-bill it, which means it’s…..drumroll please……completely FREE every month.
If you have PCOS or any other hormonal imbalance causing acne, give this a try. Birth control pills may do something similar, but may also come with slight side effects.
The only side effect here is that it may make you a bit dizzy if you stand up fast after taking it during the first few weeks, but your body will quickly learn to compensate and the dizziness will go away. So will your acne! Amazing.

This has been a miracle pill for me. I have taken it for 6 months and after 3 months have had zero hormonal acne. It’s a HG. No side effects and doesn’t dry out skin. I take 50 mg 2x a day.

I’m a week and a half in to taking this medication and it has already been a miracle worker.
My skin history: 31 with problematic skin most of my life. Thankfully I’ve never had any cystic acne, but I’ve had persistent daily breakouts that don’t seem to respond to much of anything. I’ve tried retin-a, differin, azaelic acid, salicylic acid, glycolic acid, the caveman routine, an extremely simplified routine, etc. You get the point. I’ve tried EVERYTHING including changing my diet. I went raw vegan for months even. The only relief I seemed to get was from benzoyl peroxide, which I would have to slather completely over my face every single night over top of a thick moisturizer. It cut down on the breakouts but didn’t stop them, and even then my mostly clear skin just didn’t look good. I had superficial redness, and large pronounced pores on the sides of my nose and forehead that were always very noticeable and were just amplified in appearance by my oiliness, which was extreme. I’m talking, I could wash and moisturize and within 2 hours I would be completely covered in oil.
As you can imagine, at 31 having skin like this is SUPER embarrassing. Waking up with skin that looked okay-ish and then coming home and looking in the mirror and realizing at some point I had developed 4 huge whiteheads on my chin and a pimple next to my nose really crushed my confidence. I was scared every time I looked in the mirror.

My skin is no longer red and irritated looking, my pores are no longer inflamed. Everything just seems to be calming down.
I’ve had some lightheadedness and thirst, I also woke up once or twice with a headache that felt like it was due to dehydration so I have to be more cautious of that, but overall no side effects I can’t deal with.
Also, re: the few reviews that say being on this long term has aged them, I’m taking those with a grain of salt. There’s so many factors that go into aging it’s impossible to say they would have aged any better off of this medication.
**EDIT**I’m coming up on 3 months on this medication and figured I should post an update. In regards to my skin, my improvement definitely hasn’t been linear. There have been good patches and rough patches but there HAS been improvement and I can definitely see the light at the end of the tunnel. I have also developed an annoying side effect of being terribly itchy, particularly on my scalp and my back (especially where my bra sits). I think this is due to my skin being considerably drier in those areas than it ever has been before. I’m washing my hair way less often than I used to and when I do it’s because the ends have gotten grimy and not because my roots are oily.

A few months after turning 23 I developed horrendous, painful cyst acne all over my face. Previously I’d have a pimple here or there but this time it didn’t go away and it quickly consumed my life. I went to one dermatologist and they put me on doxycycline and It did nothing. After three months at that derm, my acne got worse and I was getting extremely depressed since i as trying everything, and it was just getting worse. I finally changed up my dermatologist and the first thing the new derm did was put me on this daily. After a solid three months of this daily my acne stopped coming in and what was on my face began to clear up. This in itself is a miracle and my skin & scalp look fresh. Spiro has changed my life, I wish I had found this sooner. I’ve dealt with cystic acne since my mid teens – I’m 35 now and have tried everything short of Accutane. I was beyond frustrated, and the entire experience did a number on my self esteem. I don’t think people who haven’t experienced this really understand how awful it is. Anyways, six months ago I begged my doctor to put me on Spiro and he did. This is the dosages I have been on:
PRE- SPIRO: Always at least 2-3, sometimes more, giant cysts on my face, usually on my chin or near crease of nose. They would last for weeks only for one to go away and be replaced with a new monster.
MONTH 1: 25mg/day – Skin less oily but not any noticeable improvement.
MONTH 2: 50mg/day – Skin less oily, still getting 1-2 cysts a week but they are smaller and last 2-3 days as opposed to 3 weeks.
MONTH 3: 50mg/day – Skin less oily, 1 pimple/week where cysts used to form. Still have some small blackheads that like to start around nose crease.
MONTH 4: 50mg/day Maybe 1-2 small pimples/month on chin. Wouldn’t classify skin as oily anymore but still have small blackheads that form near nose crease. Pores look smaller.
MONTH 5: 100mg/day Zero acne, non-oily skin, pores definitely smaller…my self esteem is so much better.
MONTH 6: 100mg/day Still no acne, non-oily skin
SIDE EFFECTS: My blood pressure is normally on the low side and I have to make sure I consume A LOT of water and eat enough salt or I can feel light headed. This medication WILL dehydrate you, so you need to prepare accordingly. Recently, I started taking an electrolyte tablet daily with 600 ml of water (in addition to LOTS of plain water), and I feel great. I feel that with the increased dosage of 100mg/day, I need to take this electrolyte. I also try to limit my potassium intake. I have not had any blood work done to check to see if my levels are good, but plan to request to in the next month or so. My DR wasn’t worried about it, as I felt good.
I’d recommend trying Spiro if you battle the kind of acne that I did. Upping my water intake and having to take an electrolyte pill daily, along with this medication is a very small price for clear skin. Dairy and caffeine are acne triggers for me, and I continue to avoid consuming them. I’m so glad I found something that works and can only hope it continues to do so.
